Insurance & Billing

Thank you for choosing Cardiology Associates of Green Bay. We understand that billing and payment for health-care services can be confusing and complicated. We are here to assist you with all issues regarding your insurance and billing.

We participate with most insurance companies as well as Medicare and Medicaid. Please bring your insurance card and identification with you on the day of your visit. You will be registered at the time of your first visit. Some insurance companies require that you receive a referral from your primary care provider before you can be examined by a specialist. It is your responsibility to obtain this referral from your primary care provider.

If you are a returning patient, please inform us of any changes in your personal or insurance information. The staff will ask you for this information at the time of your visit. The lack of correct information may cause payment delays or insurance denials that may ultimately leave you responsible for payment.

Co-payments and balances you may owe are due on the day you receive services. If you have questions regarding your co-payment or deductibles, please contact your insurance company. For those patients with large deductibles, limited benefits, or for those who are without medical coverage, our billing department will be happy to create a payment program for you. We do not wish anyone to be denied medical care because of a financial hardship. If you have any difficulty understanding or paying your bill, we encourage you to contact our billing department. Cardiology Associates of Green Bay accepts cash, checks money orders, VISA and MasterCard.
